Making Your Process Easier to Manage


These forms are very helpful in ensuring that all your disciplinary forms are standardized and that all relevant information is obtained throughout the disciplinary process.

All relevant parties must sign where applicable, and if an employee refuses to sign the form, you can either choose to write - employee refuses to sign form, or you can elect a representative to be witness to employee refused to sign form.



Standardized Disciplinary Forms


These are the five forms you are able to download:

  1. Job Performance Counseling
  2. Disciplinary Warning
  3. Notice of Possible Dismissal Meeting
  4. Possible Dismissal Meeting Decision
  5. Appeal Request

It is important to keep copies of all completed forms on the employees file. Although the following time periods apply for the warning to remain active on file:

  • verbal warning - 3 months
  • written warning - 6 months
  • final written warning - 12 months

it does not mean that you should remove the warning from the employee's file and discard. The warning remains on file, but its not taken into account in progressive disciplinary action, if the time period has elapsed.
